I want to include a survey page with questionaire on a website. Kindly point us to a relevant template or module.

Hey,

To include a survey page with the questionnaire, you can simply create a Google form and embed the code directly into the TemplateToaster. You can follow these steps:
1. Create a Google Form. You can use this link to create the form: https://docs.google.com/forms/u/0/. Go to this link-> Click on Blank
2. Enter the Heading, Form Description, and Questions according to your requirement.
3. Click on Send-> Send via: select (< >) tab-> Copy  the code below "Embed Html".
4. Open TemplateToaster-> Double click in the content section-> Paste the embedded code in the content directly where you want to add form-> Preview
